This pleasant young women had difficulty breathing through the nose that has worsened over the last few years. No previous nasal trauma. She was found on exam to have bilateral nostril collapse (external nasal valve collapse) when she breathed in. The end of her septum was also deviated to the left. She underwent a septoplasty and an open rhinoplasty with placement of bilateral, but asymmetric spreader grafts (R>L) and bilateral batten grafts. A columellar strut and bilateral submucous resection of her inferior turbinates was also performed.

She did well postoperatively and is very happy with her results.
