What is a Deep Plane Facelift?

A deep plane facelift is a technically demanding, advanced surgical procedure that lifts and repositions the facial skin, fat, and connective tissues into a more vertical direction. Rather than simply tightening the skin, this technique works by releasing the ligaments that tether the cheek fat, allowing the cheek volume to be repositioned without tension as a single, continuous unit. This adds volume over the cheekbone and removes the volume under it to restore the beautiful cheek curves of youth. This recreates a natural, continuous, smooth, beautiful contour from the eyelid to the jawline, erasing the shadows, depressions, and irregularities of age. Repositioning these tissues without tension minimizes many of the common complications of traditional facelifts, including windblown look, pixie ear deformity, stretched scars, and sweep deformity. Deep plane access allows the improvement all the way to the deep nasolabial folds and provides a natural lifting of the sagging cheeks, jowls, and neck. This is why a deep plane facelift offers smoother, longer-lasting results, often maintaining its effects for 10 to 15 years or more.

What is the Difference Between a Vertical Vector Facelift and a Deep Plane Facelift?

A deep plane facelift is the surgical procedure required to access the deeper layers of the face through incisions around the ear, extending to the side of the nose, and from the lower eyelid down into the neck. This maintains the skin with its deeper support structure for strength and improved skin health. In the deep plane, ligament release can be performed, allowing the facial fat and skin to be restored and moved as a single, continuous unit. Vertical vector facelift refers to the preference to move this continuous unit in a more vertical direction, against the pull of gravity. This is in contrast to the horizontal movement of a traditional facelift. The Deep Plane Facelift and Vertical Vector Facelift are often used synonymously, as cutting-edge surgeons using this technique release the ligaments and lift in a 60-70 degree angle.

Who Are the Best Candidates for a Deep Plane Facelift?

A significant advantage of this procedure is that it caters to a wide range of patients across various age groups. Dr. Compton has successfully operated on patients ranging from the age of 30 to those in their 80s, as different individuals are concerned with the signs of aging at various points in their lives. Many individuals benefit from this procedure because they are experiencing moderate to severe facial sagging, loss of volume, and issues with skin laxity. Others want long-lasting results after tiring of using dermal fillers continuously. Regardless of the reason, it’s important that patients are in overall good health, don’t smoke, have not had Sculptra filler, and have realistic expectations for this procedure. While a deep plane facelift delivers impressive anti-aging results, it won’t halt the ongoing natural aging process, so ongoing maintenance with skincare, toxins, Morpheus, and lasers will still be beneficial.

What Are the Benefits of a Deep Plane Facelift?

Deep plane facelifts are the gold standard for facial rejuvenation results because of the unrivaled benefits. Since this procedure tackles signs of aging from a foundational level, the effects can last well over a decade. It’s especially great for addressing severe signs of aging, such as jowls, loss of jawline definition, sagging neck, deep wrinkles, and loose skin. There is no substitute for repositioning the facial fat to its natural position to restore volume to the correct position and size, rather than filling the loose skin with hyaluronic acid. Many individuals who had previously relied on fillers to rejuvenate their appearance find that they start to look puffy or overfilled, as the wrinkles are filled out. These patients often opt for deep plane facelifts, as they provide a long-lasting and natural solution to an aging appearance.

What’s the First Step?

To determine if you’re a suitable candidate for a deep plane facelift, consult Dr. Compton at his Troy, MI, office. During this consultation, Dr. Compton will assess the severity of your skin laxity and identify areas of concern, including sagging skin, wrinkles, jowls, and excess fat versus fat loss. What to Expect on the Day of Deep Plane Facelift Surgery

Once you arrive at the designated location, preoperative preparation will begin. Dr. Compton and his surgical team will review the surgical plan with you one more time to confirm the correct list of procedures and any specific preferences. Dr. Compton will spend time meticulously marking the face for mapping and incision planning. From there, you will head to the operating room, and we will ensure you are in a comfortable position, pressure points padded, and the anesthesia team will gently sedate you. All nationally recognized surgical protocols will be followed, including a time-out to review your case details. After numbing and sterilizing the face, Dr. Compton will begin the surgery, which typically lasts between 3 and 6 hours. You may have a catheter inserted once asleep, and you may have drains in place after the surgery. The incisions are closed with sutures, and your hair and face will be washed.

After surgery, you’ll be closely monitored before you can be discharged. Most patients are discharged on the same day; however, an overnight stay in the hospital may be required if the procedure is particularly complex or lengthy. You will follow up in the office the very next day to ensure recovery is proceeding smoothly. Dr. Compton will ensure that you have everything you need in post-operative care instructions to facilitate a safe and comfortable recovery.

Deep Plane Facelift Recovery

In the first week of recovery, expect swelling, tenderness, and bruising around the cheeks, jawline, and neck. To help minimize swelling, we recommend keeping your back elevated to a 30-degree angle while maintaining a neutral position for your neck. We do not want your head flexed forward as this limits the outflow of swelling through the neck. We do not recommend wearing compression garments. You will be taking antibiotics and steroids, and will be prescribed anti-nausea and pain medication for your comfort. You will have multiple follow-up appointments during the first week to closely monitor healing and avoid complications.
