20 yo male with a deviated nose and twisted, fractured septum. He had a nasal fracture in 7th grade and had difficulty breathing which interfered with both sleep and exercise. A preoperative CT scan also discovered bilateral concha bullosa (air filled middle turbinate).

He underwent an open rhinoplasty with major septal reconstruction, bilateral spreader grafts (L double, R single), columellar strut, asymmetric osteotomies. His inferior turbinates were reduced with powered inferior turbinoplasty and the middle turbinate concha bullosa were reduced.

He is breathing great and very happy that his nose is now straight.
